To apply for the ISHAM membership, please fill out the form below. The membership fees are indicated in the table. Regular members can save up to CHF 100 if they apply for a multi-year membership, Young ISHAM members receive a 50% discount on the regular membership fee and can save up to CHF 50 if they apply for a multi-year membership. In case a membership payment/renewal cannot be done via our website, please contact the ISHAM Secretariat at isham@congresscare.com.

Persons who would like to apply for the ‘Patient advocacy membership’, which is free of charge, need to contact isham@congresscare.com and provide in the email the following information to efficiently setup the membership: Name/Surname, Age, Country, Spoken languages, Invasive fungal infection (diagnosis), Diagnosed (date), as well as indicating their interested in patient advisory board and/or review of summaries and/or patient-to-patient advice, and/or other.
